Remote Opportunity Scheduled: Monday-Friday 9 am to 6 pm or 12 pm to 8 pm Central Standard Time TX candidates highly preferred. PRIMARY FUNCTION: The primary functions of the Patient Outreach Coordinator is to contact patients, parents and/or guardian to schedule appropriate visits depending on patient needs. Our team is focused on promoting good health through yearly well visits and care gap closures.
ESSENTIAL FUNCTIONS OF THE JOB: (This list may not include all of the duties that may be assigned.)
- Contact patients, parent and or guardian based on the assigned list of patients who are in need of outreach to coordinate and schedule appointments utilizing the Practice Management Scheduling module, while meeting specific patient needs and closing gaps in care.
- Review charts to ensure patient records are accurate and identify care gaps for the patients that will need to be addressed during the office visit.
- Register patients in the Practice Management System, being sure to enter their most up to date demographics and verify patient insurance using designated websites.

Adhere to all organizational information security policies and protect all sensitive information including but not limited to ePHI and PHI in accordance with organizational policy, Federal, State, and local regulations, including, but not limited to protecting patient information under HIPAA laws.

Other Physical Requirements
EDUCATION: High School or GED required.
LICENSURE/CERTIFICATION: Registered or Certified Medical Assistant preferred.
*Upon hire, and for the duration of the employment period driver’s license must be active and valid.*
EXPERIENCE: up to one-year experience in patient engagement, advocacy or outreach preferred. Up to one-year experience in scheduling preferred. Previous healthcare preferred.
Knowledge, Skills & Abilities
- Utilizing appropriate customer service skills in all encounters with patients, parent and/or guardian, professional contacts and employees.
- Strong written and verbal communication skills to include the ability to receive and convey messages electronically via EHR, Outlook and other additional communication sources as needed.
- Proficient working on the computer with excellent typing skills.
- Ability to use different computer applications such as: Electronic Health Records, spreadsheets, word documents, electronic mail, and additional systems as needed.
- Ability to prioritize workload, work independently, organize and complete assigned tasks accurately and in a timely manner.
- Demonstrate skills in effective written and verbal communication.
- Follow a defined list of tasks and complete them in a timely fashion.
- Follow established policies and procedures.
- Be a team player.
- Ability to multitask, move from one assigned task to another with ease.
