Your Role

As the Clinical Program Manage you will be responsible for helping develop and operationalize key components of the Honest Health clinical model. You will support the building and evolution of clinical and operational infrastructure for Honest programs to be implemented across both central and market functions, working closely with cross-functional stakeholders for development insight and proper execution. The Manager, Clinical Program Management will work directly with market clinical supervisors to ensure the Honest clinical model is leveraging appropriate and innovative KPIs to continually drive for the best possible patient outcomes. The Manager, Clinical Program Management will support the overall business by ensuring the care management team has the support they need to deploy the clinical model and will be responsible for providing support to the market-level clinical leadership to ensure the delivery of high-quality care.

 

Primary Functions Include:

  • Partner closely with market-level clinical leadership to ensure the successful deployment of evidence-based, best-in-class care management programs in a multidisciplinary, telephonic and home-based care model.
  • In conjunction with market leadership, be the subject matter expert for the tools and systems needed to support care management activities.
  • Support market leadership in training new and existing care management team members through a comprehensive education and training program.
  • Help support local clinical leadership in the deployment and ongoing oversight of the clinical and operational workflows within applicable Honest systems.
  • Closely integrate with market clinical leadership to support operational execution and clinical enablement of the clinical workflows and associated tools.
  • Continually review internal workflows and management processes to drive increased effectiveness of programs and services.
  • Establish a culture of excellence in performance and innovation that optimizes opportunities to create new processes that better serve patients and their families, with the goal of reducing medical spend and enhancing patient quality outcomes.
  • Support market clinical leadership as they monitor metrics and benchmarks to focus the work and drive collective improvements in care.
  • Work closely with the Clinical Education team to ensure education and training continues to meet evolving team member needs. Identify team member knowledge, skills, and ability need and determine resources to further enhance their professional growth.
  • Assist with the development and implementation of care management policies/procedures, workflows, job aids, and protocols and review/revise annually and at other times when needed.
  • Understand STAR and HEDIS measures and standards. Assist in the development of a plan to close gaps, as needed.
  • Perform other related responsibilities as assigned.

  • Must have reliable access to high-speed internet to ensure seamless remote work communication and productivity
  • Current, unencumbered license as a Registered Nurse
  • Bachelor of Science in Nursing from a nursing accredited school of nursing required; Master of Science, Doctor of Nursing Practice, or Doctor of Philosophy in nursing preferred
  • 4+ years of progressive experience in healthcare management, operations, and program development
  • 2+ years of clinical experience in home care, health plan or hospice within the previous 10 years
  • 2+ years of care management experience within the previous 10 years
  • Experience with integrated delivery systems and principles of value-based care
  • Experience assisting with the development and implementation of innovative clinical programs with clear demonstration of results
  • Strong understanding of multidisciplinary team management
  • Knowledge of HEDIS measures, quality gap measurement, STAR ratings
  • Expertise in using data, metrics, and user feedback to drive strategy and execution
  • Solid computer skills including the Microsoft Office Suite (Word, Excel, PowerPoint) and Visio (preferred but not required)
  • Experience with Medicare ACO Medicare Advantage plans preferred

The base pay range for this role is $94,300.00 - $108,400.00 Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, and organizational needs. Base pay is just one piece of the total rewards program offered by Honest. Eligible roles also qualify for short-term incentives and a comprehensive benefits package.
