Summary of Job Duties:
This position works under supervision of the Population Health Improvement Manager on site or a member of the IMSL Quality Team. The Population Health Improvement Assistant will perform administrative duties and documentation in support the completion of Population Health, Value Based Care, and Quality objectives. This will include patient outreach and engagement, pre-visit planning work, data tracking, or other coordination or administrative processes needed to meet value-based requirements.

Qualifications:

Minimum Qualifications:

  • High School Diploma, GED or formal educational equivalent

  • ​Preferred: Associate degree in Health Information Management, Business, Nursing or Other health related field preferred.

  • 2 years’ experience as a Medical Assistant, Scheduling/Access, HIM, or other clinical or support role.

  • Good communication skills, computer/basic keyboard skills, spreadsheets, telephone etiquette skills, or general knowledge of office machines including printers and scanners.

Preferred Qualifications:

  • Associate degree in Health Information Management, Business, Nursing or Other health related field preferred.

This position supports the clinics administrative and clinical operations through: panel management, data-collection, documentation for quality work, Care Gap Closure, and supports the Population Health Improvement Manager (PHIM) set or implement standard work and best practices.  

  • Assist with Population Health and Value Base Care Quality 

  • Assist the PHIM and Quality Team in documenting outcomes and activities related to Population Health and Value Based Care programs when data entry is required in the EMR or Payer Platforms

  • With clinical and Quality Team input, assist on the identification of high-risk patients with chronic care needs and meet any special documentation required for payer’s value-based care 

  • Collect data and documentation needed to support quality improvement work, clinic operational needs, and internal audits. 

  • With the direction of the Quality Team, use standard workflows to review payer reports, compare to Epic data, and act to complete payer required documentation or audits.

  • Provide administrative support for patient’s pre-visit to ensure visits are successful and effective 

  • Support any administrative activities related to quality, patient experience, and safety.

  • Help review and address issues of accessibility: appointment availability, well visits, ED utilization, MyChart, and other access related measures and use standard work process to address needs identified

  • Perform outreach to patients to attempt to engage them in their care and get the patient in for care or connected to community resources if indicated. 

  • Communicate with care team members as needed with patient needs when pre-visit needs are identified.

  • Receive, reconcile, and utilize patient lists from external sources to help provide outreach, care gap closure, and outcome improvement when possible. 

  • Supports meetings, planning, and education related to Quality 

  • Assist with preparing agendas, meeting materials, and gather input from attendees.

  • Record, maintain, and distribute minutes for meetings.

  • Support follow-up on agenda items between meetings

  • Assist coordination of meetings related to Patient/Family and community continuum of care.

  • With assistance of the care team, keep educational materials up to date and physically accessible for staff and patients 

  • Support the PHIM and Quality team in running reports and putting them into a standard visual display and place them in designated locations.

  • Other duties as assigned.

Demonstrates effective communication skills; communicates accurate and complete information; maintains strict confidentiality when necessary. Demonstrates positive working relationships with co-workers, management team, and ancillary departments; follows the Core Concepts of Patient and Family Centered Care and the Basic Code of Conduct Guidelines. Assist internal and external stakeholders as needed with exceptional customer service.
