Required:
Experience
Required: One year of accumulated LVN experience. LVN with less than one year’s experience may apply but will require agency to get state exception.
Essential
Participates in the development and maintenance of a Plan of Care that includes problem identification, goal setting, planned intervention and an ongoing evaluation process.
Encourages other staff and family to participate in home care planning and communicates information.
Implements and adheres to physician's orders where such orders are within the legal scope of practice of the skilled nurse.
Performs skills and techniques based upon current theory and practice.
Evaluates the patient's nutritional status, habits and potential for improvement.
Provides safe nursing care and exerts every reasonable effort to influence environment safety. Uses equipment correctly and identifies supply needs.
Uses all possible precautions when administering drugs.
Provides appropriate and professional documentation of patient care.
Responds to emergencies in an efficient and effective manner.
Additional
Performs general nursing measures including those that comfort and soothe the patient.
Demonstrates the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area. Knowledgeable of growth and development for all patient/family cultural, linguistic, spiritual, gender, and age specific needs. Able to effectively communicate and care for patient and family as reflected in the Plan for Provision of Care.
Performs other duties as assigned.
RESTRICTIONS: LVN'S IN THE HOME MAY NOT:Draw blood unless certified by their state to do so. Start IV's or handle IV fluids. Administer hyperalimentation/TPN solution. Irrigate a central venous catheter. Irrigate a heparin lock. Administer any IV medication by IV push. Insert a nasogastric tube unless certified by the state to do so.
