Overview:

We are hiring for a Stroke EMS Outreach Coordinator for UofL Health's Comprehensive Stroke Center.

Location: 530 South Jackson Street, Louisville, KY, 40202

Shift: Salaried, 40 hours/week, Days

 

Job Summary

This employee develops and strengthens relationships through contacts in Emergency Medical Services (EMS) and referral facilities and identifies and develops additional referral markets. The employee designs and implements education and screening programs for community presentation and designs and implements an effective primary care, referral source, and EMS feedback program to enhance continuity of care and strengthen referring relationships.  Employee will be very active with regional EMS agencies, communities, and referral healthcare organizations. This employee works under the direction of the service line leader, works collaboratively with all members of the system stroke team, utilizing established structures and processes, supporting the goals and initiatives of the service line, education, and the organization. This employee works collaboratively with marketing and other service lines to deliver targeted referral development and community education offerings.

Responsibilities:
  • Works with Stroke System of Care Director and Medical Director in assessing community, EMS and outlying facility referral development opportunities
  • Works to broaden UofL Health’s stroke referral base relationships such that stroke referrals increase year over year by at least 1%, based on identified opportunities and innovative strategies implemented
  • Serves as a resource for outlying facility education/prevention and quality improvement; assists with program development as requested
  • Collaborates with program assistant, APRNs, physicians and the system stroke team members to provide follow-up contact with referring physicians/facilities, primary care and EMS as indicated through written and verbal communication
  • Establishes, leads and executes an annual calendar of “signature” events sponsored by UofL Health
  • Tracks and documents all internal and external outreach events undertaken by the stroke program as documentation for TJC surveys at each UofL Health stroke center
  • Analyzes trends in outreach offerings for efficacy in supporting the stroke program and prioritize participation in those yielding the greatest reward for our program while respecting clinician time
  • Establishes and executes a system for providing feedback to PCPs and other relevant stakeholders in our patient’s care on a timely basis
  • Establishes, leads and executes a multi-pronged EMS outreach program
  • Works collaboratively with facility-wide team outreach team to coordinate efforts to maximize UofL Health’s outreach impact and efficiency across the organization
  • Engages with direct care staff as well as physicians and program leadership in outreach efforts to increase their sense of ownership in the stroke program
  • Markets UL Health services professionally throughout the region
  • Maintains compliance with all company policies, procedures and standards of conduct
  • Complies with HIPAA privacy and security requirements to maintain confidentiality at all times
  • Performs other duties as assigned
Qualifications:
Required: 
  • Bachelor Degree in Nursing, Paramedicine, or related healthcare degree 
  • Minimum of two (2) years of experience as a RN or paramedic 
  • Prior experience as an RN or Paramedic in an ED, ICU, and/or EMS setting
  • Active Kentucky RN license, or compact license with privileges to work in Kentucky, and/or Kentucky Emergency Medical Technician (EMT)-Paramedic license
  • Basic Life Support (BLS) for Providers accredited by the American Heart Association (AHA)

Preferred: 

  • Experience in education and/or teaching
