Summary:
The Addiction Counselor performs client-oriented activities and related professional services in caring for the intellectual, psychosocial, physical and spiritual needs of the patients. The Addiction Counselor is directly responsible to the Clinical Services Manager.
Job Duties & Responsibilities:
Provide addiction services care to assigned patients.
- Interact with patients, offer one-on-one sessions, facilitate individual goals to address psychosocial, spiritual, intellectual, and physical need.
- Follow the interdisciplinary treatment plan. Report and record patient’s behaviors, interactions, response, and progress toward treatment goals.
- Monitor behavior and intervene to ensure dignity, learning and well-being of all patients, and reinforce behavior through interactions based on program philosophy.
- Understand and implement daily patient activities specific to the assigned program to assist in overcoming addiction, adjusting to life, and making changes.
- Provide a group treatment setting in which to address the issues relating to drug and alcohol problems.
- Perform case management response to assigned patients.
- Use knowledge and skills to collect and review information regarding the patient’s status.
- Complete addiction assessments as required.
- Identify age-specific and culturally specific needs.
- Provide interventions that are appropriate to the age of the patient population served.
- Modify treatment activities and approaches as needed to comply with changes in patient’s status.
- Facilitate individual, group and family sessions regarding addiction issues, as needed.
- Serve as a case manager for addiction patients as assigned.
- Initiate and maintain collateral contracts.
- Coordinate therapy planning and discharge staffing.
- Coordinate aftercare plans.
- Advocate for patient rights.
Complete assignment and documentation within required time.
- Collect and record data for use in developing treatment plans.
- Document patient’s treatment status and progress (medical record progress notes) using approved method under the supervision of assigned staff. Report and record patient behaviors, interactions, responses, and progress toward care goals.
- Document patient’s participation progress in medical record.
- Report and document suspected cases of physical, sexual abuse and/or neglect per Wisconsin law and according to hospital policies and procedures.
- Provide clinical reviews to patient care specialist as requested, to assist in the utilization review process.
- Maintain confidentiality of all health records, adhering to all state and federal regulations.
Interact and communicate professionally with the addiction treatment team.
- Communicate, verbally and in writing, with other team members regarding patient schedule, programs, progress, behavior, and specific incidents.
- Attend staffing and related meetings as directed.
- Participate in team meetings and staffings, providing consultation.
- Attend all mandatory in-services.
- Contribute in a positive, solution-focused manner.
- Demonstrate effective communication skills and good organizational skills that promote timely response to all inquiries and task completion.
Participate in Rogers Behavioral Health’s performance improvement monitoring and evaluation activities, as requested.
Maintain environment in a safe and orderly manner. Take all necessary precautions and observe all safety rules to prevent injury to patients an assure hospital resources are secure.

- Comply with the Hospital policies and procedures, including Human Resources, Infection Control, and Employee Health policies and programs.
- Project a professional image by wearing appropriate, professional attire.

Physical/Mental Demands:
- Be flexible with work assignments, including schedule changes, changes in required documentation, program additions, and changes in policy and procedure.
- Be accessible to patients, show support, and conduct appropriate interactions. Display proper judgment and respond in a proactive and non-intrusive manner, regardless of a patient’s behavior.
- Be able to work cooperatively and supportively with all staff. Show courtesy, respect, and willingness to work out problems on a one-to-one basis.
- Position involves exposure to aggressive/combative patients. Must be able to successfully complete aggression control training.
- Verbal and hearing ability is required to interact with patients and employees. Numerical ability required to maintain records and operate computer.
- Be able to plan, control and direct all aspects of employee relations. Tact is required to deal effectively with employees and professional staff. Logical thinking and discretion are required to make decisions in initiating and implementing policies and procedures and standards.
- Must be able to read and communicate through written, verbal, and auditory skills and abilities.
- Be physically/mentally able to perform job duties as verified by a physical exam by a licensed physician, per post- employment physical.
Education/Training Requirements:
- Valid Wisconsin Certified Substance Abuse Counselor, Substance Abuse Counselor, or Substance Abuse Counselor in Training (CSAC, SAC, SAC-IT) required.
- Current American Heart Association CPR certification or American Red Cross Professional Rescue is required upon hire. Bi-annual re-certification is required.
- Annual aggression control training is required.
