The Consultative Pharmacist 2 consults with patients, pharmacies and providers to provide patient care in the form of optimal medication therapy. The Consultative Pharmacist 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.
Schedule: Monday through Friday 2:30 – 11pm EST (1:30 – 10pm CST) with a 4th weekend rotation
Location: This will be a work at home opportunity based out of Pennsylvania or Tennessee. Candidates who live in New Jersey will also be considered but must work onsite in Philadelphia.
Essential Duties and Responsibilities:
Clinical Consultations
- Recommends appropriate pain and symptom management therapies based on patient specific information
- Provides real-time collaboration with nurses to create a cost-effective medication care plan
- Focuses on medication misadventure prevention by providing education to nurses and caregivers
- Enhancing patient’s quality of life
Documentation
- Advances patient care by documenting interventions for outcomes measures
- Maintains accurate patient profiles
- Verifies encounter information is complete before submitting to be transcribed/processed
- Produces quantity of work that is at or above the documented average
- Possesses excellent communication/interpersonal skills

Education and/or Experience
- PharmD or B.S. in Pharmacy from an accredited school of pharmacy required

Pay Range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$93,000 - $128,000 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
