Position Purpose: The Medication Therapy Management Clinical Pharmacist provides specialized review of patient profiles and makes recommendations for modifications of drug therapy.
- Evaluates medication profiles to determine if changes are warranted to optimize the member’s drug therapy and minimize the potential for adverse drug reactions.
- Recommends alternative medications, alternative dosing, or other modifications (including discontinuation of medications) as appropriate.
- Reviews therapy plans with physicians as needed.
- Perform interactive, person-to-person, telephonic comprehensive medication reviews with members.
- Works in a multidisciplinary team environment to develop and enhance MTM program and the software used to manage the program.
- Other duties as assigned.
Education/Experience: Bachelor’s or Doctorate of Pharmacy degree. Minimum one year pharmacy residency training completed (or equivalent training/experience) recommended. 1-2 years of experience working within a pharmacy or related health care environment required.
License/Certification: Current, valid & unrestricted state Pharmacy license required and must maintain. Based on work assignment, may be required to obtain additional Pharmacy licensure in multiple states within six months of employment.
Pay Range: $103,500.00 - $191,600.00 per year
