The Job
The Patient Care Coordinator will support SCAN Mobile Care Team’s clinicians and coordinate care for SCAN members to achieve its mission to enhance seniors’ ability to manage their health and live independently.
You Will
Work closely with SCAN Mobile Care Team (SMCT) clinicians to deliver an excellent healthcare experience to our members.
Support SCAN SMCT by effectively performing administrative responsibilities with minimal supervision including:
- Refill approved prescriptions, communicate with vendors, order patient medical equipment, process lab orders, and fill out patient forms.
- Schedule specialty appointments, coordinating transportation.
- Answer telephone calls in a professional manner and address/direct calls appropriately.
- Sort and distribute incoming mail and faxes.
- Maintain inventory of supplies and materials needed by clinical staff; stock and order supplies as necessary.
- Collect and organize patient medical information and reports while maintaining confidentiality and protecting patient privacy.
Support SMCT with fulfilling patient care initiatives.
Seek out opportunities for improvements from trends in patient feedback and propose viable solutions in team settings or with leadership.
Occasionally join SMCT clinicians in communities around Los Angeles and Orange Counties to support patient care.
Monitor and address clinical quality care gaps with clinicians.
During busy enrollment periods, assist SMCT sales team with set up and break down of presentations and events at senior-living communities throughout Los Angeles and Orange Counties.
Promote the mission, vision, and values of SCAN Health Plan and serve as an ambassador when interfacing with senior-living communities.
Other duties as assigned.
Your Qualifications
- HS Diploma or GED required. Bachelor's degree preferred.
- A comparable combination of education/experience and/or training will be considered equivalent to the education listed above.
- 2+ years of related experience as a medical assistant, medical office administrator, or care coordinator.
- Direct patient care background preferred but not required.
- Experience with Medicare Advantage or long term care communities preferred.
- Ability to communicate effectively verbally and in writing
- Cultural competence and sensitivity required, multilingual skills preferred Highly organized with attention to detail
- Strong interpersonal skills, hospitality, and change agent mindset essential to build relationships with healthcare team, patients, families, and senior-living communities
- Knowledgeable and able to maintain HIPAA requirements
- Adaptable and open to change and problem-solving
