Description

The ConcertoCare Social Worker is a core member of the interdisciplinary team responsible for providing integrated medical, behavioral and social care to patients with complex health care needs. They provide clinical care to medically and socially complex patients, who live in community-based settings including assisted living facilities, adult family homes, and private homes. This position ensures that the patient’s biopsychosocial needs are adequately met through the completion of comprehensive behavioral health assessments and engaging the patient, caregiver(s), and care team in treatment planning and care coordination activities. The Social Worker is responsible for the provision of brief evidenced-based behavioral health therapies and crisis intervention as a behavioral health expert within an integrated health care model. They work collaboratively with the local care team and are key in ensuring access to equitable behavioral health care in alignment with the organization’s mission, vision, and values.

RESPONSIBILITIES

  • Supports the behavioral health needs of patients on an assigned patient panel by the established clinical practices of the behavioral health team
  • Uses evidence-based models of care, such as behavioral activation, cognitive behavioral therapy, and problem-solving therapy to deliver brief behavioral health treatment for common behavioral health issues such as depression, anxiety, coping with loss, mood management, and substance-use disorders
  • Completes and documents comprehensive behavioral health assessments of patients in a timely manner
  • Administers evidence-based screening tools for depression, anxiety, and other conditions, developing person-centered care plans to address those conditions and monitor response to treatment using measurement-based treatment to target care
  • In some settings, provides consultation to group residences (such as assisted living facilities) to ensure facility staff can recognize changes in patients’ behavioral health functioning and appropriate interventions to support the needs of our patients
  • Supports psychotropic medication management by collaborating with prescribing providers, supporting treatment adherence needs, partnering with patients/caregivers for continuous monitoring of efficacy and potential negative side effects, and coordinating with internal collaborating psychiatrist and clinical care teams to address urgent psychotropic issues
  • Participates in regularly scheduled caseload reviews and clinical team meetings; facilitates effective communication regarding patients’ behavioral health needs across all members of the clinical team both verbally and in writing
  • Facilitates treatment plan changes for patients who are not demonstrating improvement in symptoms by consulting with an assigned collaborating psychiatrist / behavioral health nurse practitioner, the patient’s primary care provider and/or members of the clinical team
  • In collaboration with the interdisciplinary team, facilitates goals of care conversation to ensure health care delivery aligns with the patient’s wishes and values
  • Provides tangible care management support and facilitates referrals for clinically indicated services including but not limited to ongoing behavioral health treatment, social services such as housing assistance, specialty care, and substance use services
  • Serves as a behavioral health subject matter expert to the interdisciplinary care team by sharing knowledge and resources to expand behavioral health knowledge, increase access to behavioral health services, and improve patient care
  • Documents patient encounters, referral activities, care coordination, and other pertinent information in the electronic health record
  • Ensures care is delivered in a manner that honors the patient’s diverse values, beliefs, behaviors, backgrounds, social, cultural, and linguistic needs
  • Develops safety plans with patients to reduce behavioral health risks and improve patient self-efficacy
  • Exemplifies core values of respect, commitment, compassion, innovation, integrity, and accountability
  • Supports complex Social Determinants of Health needs for patients
  • Coaches and guides Community Health Workers in the support of patients with Social Determinants of Health needs
  • Provides clinical supervision to Social Workers not yet independently licensed, according to applicable state guidelines
  • Other duties as assigned


QUALIFICATIONS

  • Master’s degree in Social Work
  • Current state independent licensure (LICSW) in the state of Washington required
  • 2+ years of prior healthcare / behavioral health experience required
  • Strong problem-solving and organizational skills
  • Geriatric care experience is highly desired
  • Ability to manage patient complexity and multiple clients with diverse needs
  • Ability to communicate effectively in writing and verbally.
  • Demonstrated ability to perform multiple concurrent tasks with minimal supervision and meet deadlines.
  • Ability to work in a fast-paced, dynamic environment and work well with others on a team.
  • Proficient in computer skills to include Microsoft Office Suite (Outlook, Excel, PowerPoint, Word) and EMR experience
  • Reliable internet connectivity required
  • Knowledge and ability to navigate internet-based tools and applications, and proficiency in computer documentation
  • Demonstrates a high level of professionalism.

Base Salary/ Wage Range $85,000 to $100,000 plus annual bonus. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown.
