Serves as health care coach for Finding Wellness program participants reporting to the program directors and program manager. Develops, delivers and documents sustainable goals with participants in a safe supportive environment. Targeted efforts for the WVUM employees participating in the Wellness Rewards Program.
MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Bachelor’s degree in nutrition, fitness, wellness, nursing or healthcare related field.
2. NBHWC certification or ability to obtain NBHWC certification within one (1) year of hire date.
EXPERIENCE:
1. Three (3) years of experience in nutrition, fitness or wellness coaching.

1. Engage in Finding Wellness classes to gain understanding of goals and objectives of participants
2. Schedule regular coaching appointments with assigned participants
3. Document in Epic (EHR) each interaction or attempted contact
4. Provide a safe supportive space for each participant to make sustainable and lasting change
5. Collaborate with participants to develop SMARTER goals as outlined by the Finding Wellness program
6. Partner with participants to take their health into their own hands and encourage them to add healthy behaviors tailored to their needs
7. Explore with participants their motivations and barriers in creating behavior change
8. Introduce accountability for the participants and encourage them to identify other accountability partners
9. Encourage mindfulness in participants
10. Assess stages of change in different behavioral and lifestyle areas for individual participants
11. Review participants’ plan with program directors and/or program manger.

SKILLS AND ABILITIES:
