Role Description
This position will serve as a team lead and coach for their Support Coordination team. This position will provide guidance, training, and coaching to Support Coordinators while also providing Targeted Case Management Services to a caseload. This position will support the Support Coordination Supervisor to ensure the SC team is providing quality support coordination services and documentation.

What You’ll Do
- Serve as Team Lead and Coach
- Assist the Support Coordinators with support coordination practices, rules and regulations, community resources, and vendors.
- Provide technical assistance to the Support Coordination Team, assisting with new situations and difficult cases.
- Meet regularly with all Support Coordinators to assist with support coordination enhancements and job coaching.
- Available to answer questions from team members as needed.
- Facilitate team meetings and other meetings as needed.
- Provide onboarding training to the new Support Coordinators.
- Provide ongoing follow up, coaching, and necessary training and tools to the Support Coordination Team.
- Attend regular meetings with other Support Coordinator IIIs to review trends.
- Attend 40 hours of professional development and/or training that provides helpful resources and information for the Support Coordination Team, focusing on areas of need (Interdivisional Agreements, Children’s Division, Forensic, Employment, Assistive Technology, Charting the LifeCourse, etc.)
- Support caseload needs for the SC team
- Carry a diverse, yet reduced size caseload, meeting federal and state guidelines for that caseload (see Support Coordination expectations).
- Cover caseloads when Support Coordinators are temporarily absent, at the discretion of the supervisor.
- Provide caseload staffing support in collaboration with the Support Coordinator for critical and emergent cases such as risk of homelessness, risk of institutionalization, hospitalization, etc.
- Collaborate with the SC Supervisor for case assignments and case balancing on the SC team.
- Assist in quality assurance tasks
- Assist the SC Supervisor with reviewing and/or tracking completion of Individual Support Plans (ISP), ISP Monitoring, logging, Level of Care Assessments, and Health Risk Screenings.
- Provide team reports and reminders for upcoming deadlines.
- Review a random sample of consumer master files each month to ensure documentation is meeting requirements and quality standards.
- Review administrative transfers to determine all necessary documentation is present in the file.
- In the absence of the SC Supervisor, the SC III will complete the following SC Supervisor duties:
- Supervise and lead the Support Coordination Team.
- Monitor employee assignments to ensure accuracy, completeness, timeliness, and comply with CSRs, DORs, Guidelines or Directives.
- Recommend and conduct training for staff of new processes and enhancements.
- Pre-review of Utilization Review packets and/or budget authorizations before submitting to a covering supervisor.
- Provide guidance and resources to the Support Coordination Team to find solutions when problems arise.
- Organize caseloads in an effort to assure equitable caseload distribution.
- Facilitate team meetings.
- Foster an open environment for the exchange of ideas by being open to differing opinions and ideas.
- Make timely and consistent decisions and communicate those to staff in a clear manner.
- Perform other duties as assigned.
Essential Functions
- Knowledge of DD/DMH vision, mission, and values.
- Follow Code of State Regulations (CSRs), Departmental Operating Regulations (DORs), Division Directives and Division Guidelines.
- Communicate verbally or in writing with individuals, providers, co-workers, and/or supervisors utilizing agency forms, computer programs and email as required.
- Knowledge of Microsoft Office, Cimor, and Adobe Acrobat. Adaptable to updated technology and programs.
- Read and follow directions.
- Must travel to/from individuals home, provider and community locations.
- Must maintain a valid driver's license.
- Attends work according to facility policies in order to ensure adequate staff coverage.
Typical Qualifications
Bachelor’s degree and 2 – 4 years of relevant experience.
