Job description

General Requirements
Transition Planner/Manager (TM): Provides expertise in the clinical and logistical activities and services required for the transition from existing facilities to new facilities, reorganization, and consolidation of existing facilities, and/or opening new facilities; Facilitates the conduct of customer needs assessments to identify specific customer requirements for initial outfitting and transition. Services may include maintaining project documentation, coordinating schedules, and providing on-site supervision of move activity and coordination of paperwork and subcontracts involved in a relocation project. The TM must possess strong clinical background having experience in Military or Civilian clinical facilities with an understanding of how to interface with varying clinical departments, possess a working knowledge of the processes associated with the planning, design, and execution of health care facility construction, and be familiar with the myriad of transition activities associated with construction and renovation type projects;

Responsibilities & Duties
  • Interfacing with senior customer staff to support the specialized outfitting transition and activation activities described herein regarding the equipping of the facility, maximizing standardization of equipment and supplies, training and provisioning of the facility and relocating the staff.
  • Responsible for making strategic decisions affecting the execution, quality, scope, schedule and all activities related to transition service.
  • Responsible for assembling cross-functional teams to deliver transition requirements and objectives.
  • Responsible for developing and carrying out Transition and Move plans, Concepts of Operations, and Post Occupancy plans.
  • Responsible for controlling and monitoring all transition and move activities to ensure that deliverables meet scope, cost and schedule constraints.
  • Responsible for developing and providing written and oral presentations.
  • Responsible for the safe performance of onsite operations by all staff and subcontracted personnel.
  • Responsible for developing and managing the project’s transition schedule and specific departmental.
  • Knowledge and familiarity with standard Microsoft Suite Computer programs including MS Project, Microsoft Visio.  Experience with Legacy DoD/VA programs.
  • Knowledge and experience with healthcare organizations, facilities, and services that they provide.
  • Knowledge of medical operations, equipment and supplies required to run medical facilities.
  • Understanding of budget management and skills to control costs and report budget performance projects.
  • Understand a facility’s transition process and be able to perform transition planning and execution activities.
In addition to the roles listed, the TM must have a clear understanding of policies and procedures affecting the government contractors roles in supporting the customer, to include the Federal Acquisition Regulations (FAR) and the respective agency’s supplements (DFAR,VAAR), EM 381.1 USACE Safety and Health Requirements, VA Directive 7701 and 7701.1 Comprehensive Occupational Safety And Health Program, National Fire Protection Association (NFPA) guidelines and codes, Americans with Disabilities Act of 1990 and associated amendments and the Unified Facilities Criteria (UFC).
Education and Experience:
Transition Planners/Managers, at a minimum, shall have the following qualifications:
  • A four-year college degree in a healthcare or business related field from an accredited college/university.
  • 5 years transition planning experience
  • 3 years transition planning experience on relevant Federal, DOD, and/or Army projects
