Care Coordination Specialist

We founded Heyday Health because we saw the health system leaving our elderly loved ones behind and were driven to create a better solution. Our practice is being built from the ground up with the mission of transforming how, where, and when we deliver care to achieve exceptional outcomes.

We are looking for a Care Coordination Specialist to join our team of patient advocates and providers to promote seamless transitions of patient care across the Clinical & Clinical Operations teams. The role will include: 

  • Sharing responsibility with the RN Care Coordinator (RNCC) in monitoring Admission-Discharge-Transfer (ADT) feeds and clinically reviewing documentation of  utilization events.
  • Updating and maintaining internal patient tracking systems around utilization events and transitions of care
  • Ensuring that transitions of care are seamlessly completed in a timely fashion and are appropriately documented to provide exceptional patient care by assisting RNCC and Care Teams with scheduling Transitional Care Management visits
  • Reviewing and uploading documentation of Emergency room, Hospital or Nursing facility visits and appropriately updating the patient’s electronic medical records. 
  • Assisting Engagement Specialists in with telephonic outreach to newly assigned patients to schedule initial visits after being seen in an Emergency room, Hospital or Nursing Facility.
  • Assisting the RNCC in coordinating care for high risk patients 
  • Assisting Clinical Support with coordination of high priority referrals by reaching out to specialty offices to schedule consultations and obtain visit notes or testing results.
  • Providing in-home services for patients on an adhoc basis that require licensure (facilitated virtual visits, medication reviews, vaccinations, etc)

Qualifications

  • Valid U.S work authorization
  • High school diploma or equivalent
  • Active LPN license
  • Valid driver’s license and reliable personal vehicle that can be used for at-home patient visits
  • Comfort with using or learning how to use electronic health records, health information exchanges and other computer systems

Location: In-market based but primarily remote role

Salary: $50,000 - $54,000 per year
