Job description

**UAS Nurse (Contract, 13 Weeks):**
Conduct telehealth and in-home assessments for Medicaid beneficiaries using the Uniform Assessment System (UAS), evaluating clients health, functional abilities, and personal care needs. Document findings, assess eligibility for long-term care, and recommend appropriate services. This 13-week contract requires an active RN license, experience in assessment or case management, and knowledge of Medicaid/Managed Long-Term Care. Strong communication and interpersonal skills are essential for effective client interaction.
