Telehealth Nurse Practitioner - Temporary

Position Description Summary:

Join our team at GGH as an AWV Nurse Practitioner and play a crucial role in providing comprehensive care to senior Medicare patients. In this position, you will conduct annual wellness exams, create personalized prevention plans, and optimize the management of patients' medical conditions. What You Can Expect:

This is a full-time remote Nurse Practitioner role, offering a 5-day workweek with a schedule from 8 am to 5 pm (PST), totaling 40 hours per week. We do not offer part-time options at this moment.

Key Responsibilities:

  • Conduct annual wellness exams for senior Medicare patients via telehealth
  • Obtain and record patients' medical history, symptoms, and medication reconciliation
  • Perform various tests, screenings, and accurate coding
  • Collaborate with the Primary Care team to ensure holistic patient care
  • Maintain timely completion of charts following patient visits using EMR and/or portals


Requirements:

  • A current California Nurse Practitioner license
  • Clinical experience in Primary Care, Family Medicine, Internal Medicine, Gerontology, or Palliative Care
  • Working knowledge of chronic care management and medication options
  • Current DEA licensure/Prescriptive Authority and active CPR/BLS Certification
  • Bilingual proficiency in English and Spanish preferred
