Supportive Housing Case Manager - Remote | WFH

Job Overview

We are seeking a dedicated and empathetic Supportive Housing Case Manager to deliver vital case management to individuals transitioning from homelessness to shared, permanent supportive housing. This role offers a remarkable opportunity to enhance your professional skills and make a significant impact on the lives of those in need, while collaborating with a talented team led by the Women’s Campus Program Manager.

Key Responsibilities

  • Provide crisis intervention, thorough assessments, advocacy, and service connections for residents dealing with behavioral health challenges, alongside ongoing supportive counseling and case management.
  • Offer personalized in-home services, including facilitating house meetings, formulating and enforcing shared-housing agreements, and imparting life skills and tenancy guidance to ensure residents’ fundamental needs are satisfied.
  • Maintain meticulous records in a timely fashion, encompassing details such as HMIS system inputs, client consent forms, intake documentation, service plans, and discharge information.
  • Collaborate with the Volunteer Manager and Development Team to organize and implement special events, such as therapeutic programs and donation drives that benefit our residents.
  • Conduct outreach calls and suitability screenings for prospective program applicants.
  • Forge and maintain strong collaborative relationships with external agencies, effectively representing our organization.
  • Support and mentor volunteers and interns as required.

Required Skills

  • Excellent verbal and written communication capabilities.
  • Proficient in essential administrative tasks and Microsoft Office applications, particularly Word and Excel.
  • Strong crisis intervention and advocacy skills.
  • Ability to meet the physical demands outlined in the full job description.

Qualifications

  • A Bachelor's degree in Social Work or a related discipline is essential.
  • Preferred experience includes work in residential environments or permanent supportive housing programs. Individuals with lived experience are strongly encouraged to apply.
  • Prior experience working with vulnerable populations such as the chronically homeless, those with severe mental illness, or domestic violence survivors is crucial.
  • Familiarity with social service resources and affordable housing options in Morris County, as well as knowledge of HUD-funded projects, is advantageous.
  • A valid driver's license and a clean driving record are mandatory.

Compensation And Benefits

The starting pay rate for this position is $22 per hour. We offer a comprehensive benefits package for our full-time staff, including fully covered medical, dental, and life insurance upon hire, as well as eligibility to participate in a 401k retirement savings plan with up to a 6% employer match. Our generous PTO policy includes paid personal, vacation, and sick leave, in addition to 10 floating holidays each year.
