Job Overview

As a Licensed Mental Health Therapist specializing in telehealth, you will be an integral part of our innovative Care Team, playing a pivotal role in developing and implementing technology-enabled psychotherapeutic services. Key Responsibilities

  • Conduct assessments and diagnose both chronic and acute behavioral health conditions.
  • Collaborate with a multidisciplinary health team to create tailored treatment plans for each member.
  • Ensure a comprehensive care perspective is applied in complex case planning.
  • Perform initial intakes, evaluate patient needs and risks, and deliver therapeutic interventions through individual, family, and group sessions.
  • Connect members to community resources that facilitate access to a comprehensive range of behavioral health services.
  • Work alongside primary care providers, psychiatrists, social workers, and other professionals to discuss treatment plans and evaluate progress.
  • Offer psychoeducation to members, emphasizing mental health promotion and disease prevention.
  • Support referrals for members requiring specialty behavioral health care, advising on treatment plans until they access appropriate services.
  • Foster trusting relationships with patients, ensuring care aligns with their personal goals and values.
  • Participate in multidisciplinary case conferences to provide holistic care for all patients.
  • Educate and coach clinical and non-clinical team members on effectively supporting patients with behavioral health issues.
  • Assist with post-discharge planning to minimize the risk of readmissions.
  • Accurately document care and coordination efforts in our electronic medical record (EMR) system.
  • Contribute to the design and implementation of clinical protocols for treating behavioral health conditions within our care model.

Required Skills

  • Minimum of 2 years of outpatient experience with adolescents and adults, particularly in Medicaid, Medicare, or dual-eligible environments.
  • At least 3 years of experience in assessing, planning, and managing behavioral health care through direct clinical engagement.
  • 3 years of experience with individuals facing significant mental and chemical health challenges, grounded in a strong belief in recovery.
  • A minimum of 1 year practicing telemedicine.
  • Experience working with underserved populations and navigating socio-economic barriers to healthcare.
  • Solid knowledge of trauma-informed, evidence-based psychotherapy for mental and chemical health conditions.
  • Proficient in electronic medical record documentation and enthusiastic about leveraging technology to enhance care.
  • Strong capability to make prompt and accurate assessments of mental and behavioral health conditions.
  • Sound judgment in evaluating situations and making decisions, especially in complex or confrontational scenarios.
  • Effective collaboration skills with patients, primary care providers, mental health specialists, and support staff.
  • Familiarity with applications such as Microsoft Word, Outlook, Excel, and video conferencing tools; G-Suite experience is advantageous.

Qualifications

  • Active LCSW, LMFT, or LCPC license.
  • MFT and CPC interns are encouraged to apply.
  • Must be 18 years of age or older.
  • Adherence to all regulations concerning patient confidentiality and HIPAA.
  • Competence in electronic medical records and other technology-based systems.
