Job Overview

Join our compassionate team as a Registered Nurse Case Manager (RNCM) in a remote hospice care setting, dedicated to providing high-quality end-of-life care to patients and their families. Key Responsibilities

  • Assume primary responsibility for the care of assigned patients, executing all phases of the nursing process.
  • Deliver expert nursing care to hospice patients in accordance with established care plans.
  • Collaborate with attending physicians, team members, and other healthcare agencies to optimize patient care and resource utilization.
  • Perform thorough assessments, monitor patient conditions, and evaluate medication needs.
  • Work alongside interdisciplinary teams to craft and implement personalized care plans.
  • Provide emotional support and education to patients and their families regarding hospice care and end-of-life options.
  • Ensure continuity of care by coordinating with healthcare providers effectively.
  • Maintain accurate documentation in compliance with agency policies and standards.
  • Participate actively in interdisciplinary team meetings to address patient care needs.
  • Assess the necessity for caregiver and Certified Nursing Assistant (CNA) services, conducting supervisory visits as needed.
  • Provide scheduled on-call coverage and maintain annual clinical competencies.
  • Uphold patient rights, privacy, and confidentiality at all times.

  • Proven ability to collaborate effectively with interdisciplinary teams.
  • Proficient in assessing patient needs and executing individualized care plans.
  • In-depth knowledge of hospice philosophy and end-of-life care principles.

Qualifications

  • Bachelor's degree in Nursing (BSN) is required.
  • Active RN license in the state of Oregon is mandatory.
  • A minimum of 2 years of RN experience is preferred, ideally with prior hospice or palliative care exposure.
  • Familiarity with Memory Care, Residential Care Facilities, and Adult Family Homes.
  • Valid driver's license, car insurance, and reliable transportation are necessary.
