Position Overview

Key Responsibilities

• Deliver exceptional care and treatment to patients within their residences.
• Execute a variety of clinical tasks, including:
• Wound care and IV therapy
• Patient education regarding disease management and medication
• Telehealth consultations
• Venipuncture and Foley catheter care
• Conduct thorough assessments and formulate individualized care plans.
• Maintain accurate electronic documentation of patient care.
• Act as a bridge between patients, physicians, and external case managers.
• Perform supervisory visits for paraprofessional staff.
  

Required Skills

• Strong written and verbal communication abilities.
• Proficient in developing care plans and conducting assessments.
• Experience in patient education on medication and disease management.
• Competent in wound care and IV therapy.
• Familiarity with telehealth consultations.
• Knowledge of venipuncture and Foley catheter care.
  
  

Qualifications

• Graduate from an accredited professional nursing school.
• Valid nursing license to practice in Ohio.
• Current driver’s license.
• Ability to successfully pass a BCI background check and drug screening.

Compensation And Benefits

• Annual salary between \(57,000 and \)72,000, commensurate with experience and qualifications.
