Key Responsibilities

• Deliver customized therapy services tailored to each patient’s unique care plan.
• Guide patients through their recovery journey by providing hands-on treatment and educational support.
• Monitor and evaluate patient progress during therapy sessions, ensuring accurate documentation through our intuitive software platform (Kinnser.net), available 24⁄7.
• Serve as a key communicator, fostering collaboration with patients, their families, and our Clinical Team to ensure seamless care.
• Utilize a company vehicle to travel to patient residences, facilitating convenience in service delivery.
• Acquaint yourself with OASIS E and comply with CMS guidelines to uphold our commitment to high-quality care.
  
  

Required Skills

• Strong organizational abilities to handle patient documentation and care management efficiently.
• Expertise in delivering personalized therapy services in accordance with each patient's individualized care plan.
• Excellent communication skills to effectively liaise with patients, their families, and the Clinical Team.
  
  

Qualifications

• Degree from an accredited Physical Therapy program.
• Valid and unrestricted Virginia Physical Therapy license.
• Prior experience in home health is preferred.
