What You’ll Do

Provides supervision of social case management services
Provides coverage as needed to meet program goals
Plan, develop, organize, direct implement and monitor a coordinated approach to social case management services
The role of Manager, CHN responsibilities include overseeing the activities of the department staff, ensuring quality, productivity, functional excellence, and efficiency to accomplish strategic and operational objectives
Participates in interdepartmental groups to represent social case management needs in different aspects of Imagine Pediatrics
Establishes goals for performance standards
In addition, this position is accountable for employee engagement, development, compliance, training employees on work related performance, and developing and implementing policies and procedures to ensure a safe and effective work environment
This position also ensures training, monitoring and operations initiatives are implemented which secure compliance with ethical and legal business practices and accreditation/regulatory/government regulations

What You Bring & How You Qualify

A qualified candidate will be empathetic, caring, organized, and has strong relationship-building skills. In this role, you will need:

Master's Degree in Social Work
Certification: Licensed by the State of Texas as a Licensed Master Social Worker(LMSW) Required
Bilingual in Spanish strongly preferred
5+ years of experience 
2+ years of management experience strongly preferred
Experience in program development a plus
Experience working with a pediatric/caregiver population strongly preferred, virtual care experience a plus
