Job Overview

We are looking for a motivated Senior Case Manager to join our esteemed Case Management Department, who is passionate about improving the lives of individuals receiving waiver services. In this pivotal role, you will be responsible for verifying eligibility, creating and tracking Service Plans (SP), and ensuring the effective delivery of support services.

Key Responsibilities

  • Oversee a caseload of individuals receiving waiver services, ensuring that all required documentation is accurately maintained and updated on a quarterly and annual basis.
  • Lead interdisciplinary team (IDT) meetings to coordinate tailored services, fostering seamless communication and collaboration among team members.
  • Provide case management assistance to temporarily assigned individuals and support departmental and team coverage as necessary.
  • Mentor and train Active Case Managers, fostering a culture of professional growth within the team.

Qualifications

  • A Bachelor's Degree in a related discipline or five years of relevant experience in Long-Term Services and Supports (LTSS) focused on Developmental Disabilities; a combination of education and relevant experience will also be considered.
  • Proven expertise in service coordination or case management, demonstrating initiative, leadership capabilities, adaptability, and strong problem-solving skills.
  • A valid Colorado motor vehicle license and proof of motor vehicle insurance are required.
  • Ability to meet agency driving requirements; waivers may be considered upon request, qualifying need, and management approval.
