BA or BS degree in related field required, Minimum of two years experience with individuals with disabilities or seniors, Bilingual in Spanish, Strong Microsoft Office skills, Knowledge of Medicaid services preferred.
Key responsabilities:
- Manage a workload of 110-145 participants
- Provide training and support to participants
- Assist in enrollment and resource identification
- Monitor health and safety through visits and calls
- Document participant progress and report issues
