Major Responsibilities:

  • Provides integrated support to patients, pharmacists, and various healthcare providers by providing a “hands-on” approach to total quality patient care.
  • Works closely with patients and/or their families to obtain the best possible medication and allergy history. Includes:
  • Interviewing patients and/or their families
  • Utilizing active listening skills
  • Utilizing available resources to overcome barriers (i.e. language barriers).
  • Understanding basic medication information.
  • Integrate and collaborate with the emergency department’s admitting clinical teams, consisting of physicians, nurse practitioners, nurses, and case managers (i.e. Transition of Care Team, ED Team, Medicine Team, Surgery Team,etc.). 
  • Operate electronic health records to obtain patient medication history information.

Contact outside medical facilities to obtain patient medication history information. Includes:

  • Pharmacies.
  • Physician offices
  • Hospitals
  • Long-term Care Facilities

Document medication and allergy discrepancies in the patient medical chart.  Clarify orders with prescribers.

  • Troubleshoot technical problems with pharmacy technology and equipment.
  • Process orders using pharmacy computer system. Deliver patient medications to patient’s rooms and medication room areas as necessary.
  • Assist in training PharmD students and pharmacy residents on the medication reconciliation process.
  • Maintains required documentation in accordance with regulatory and hospital requirements and maintains respect for patient confidentiality.  Generate necessary pharmacy reports.
  • Participates in identifying problems or areas for improvement and assists supervisor with problem solving projects and improving operations; participates in Continuous Quality Improvement activities and/or teams.  Help develop new processes to ensure smooth operations and patient care.
  • Maintains a clean work area and assists in quality indicator monitoring.
  • Assists in coordinating monthly Joint Commission activities, including nursing unit inspections. Participates in the Joint Commission survey.

Position Qualifications:
License/Certification/Education:

Required:

  • High school diploma or equivalent. 
  • Current license with the Massachusetts Board of Pharmacy.
  • Certified by one of the national governing bodies such as Pharmacy Technician Certification Board PTCB, or the National Healthcareer Association ExCPT.

Experience/Skills:

Required:

  • Three years of previous pharmacy experience.
  • Position requires proficiency in math with knowledge and ability to use a computer for data entry. 
  • Must be able to follow oral and written instructions with accuracy, efficiency, and dependability.
  • Positive work ethic, motivated and professional demeanor to promote teamwork in the department.
  • Strong interpersonal communication skills.

  • Completes benefit investigations when requested by Advanced Therapeutics PharmD and APP team and appropriately documents result on patient electronic medical record.
  • Submit prior authorizations to insurance via different methods including online and telephone. Appeal decisions as necessary and conduct follow up phone calls inquiries to insurance.
  • Knowledgeable in manufacturer patient savings opportunities and communicates information clearly and effectively to providers and patients.
  • Performs follow up phone calls to patients as needed regarding insurance decisions, rejections, and prescription filling information.
  • Maintains a perpetual inventory of appropriate medications from internal pharmacies and communicates to clinic team on a daily basis.
  • Coordinates prescription delivery with patient and Home Delivery pharmacy and queues prescription up for home delivery team.
  • Maintains a clean work area and assists in quality indicator monitoring.
  • Participates in identifying problems or areas for improvement and assists manager with problem solving projects and improving operations; participates in Continuous Quality Improvement activities and/or teams.
