Job description

We offer an array of medical and chronic care services, and we’re backed by SCAN, a not-for-profit organization that has a history of supporting and improving the health and independence of older adults for over 45 years.

The Bilingual Mandarin RN Care Manager will collaboratively assess and evaluate the member’s physical health, mental health and psychosocial needs to complete individualized care plans. Conducting care plans in collaboration with the Care Coordinators to align with Health Plan partner’s Special Needs Plan (SNP) Model of Care (MOC). The Bilingual Mandarin RN Care Manager will also be responsible for supporting high-risk patients’ care management, including transitions of care coordination for health plan.

Responsibilities include:

  • Assesses members to identify medical, physical and psychosocial needs via telephonic assessments
  • Ensures appropriate assessments and care plans for this population are completed according to Health Plan SNP MOC, state and federal regulations and Homebase Medical policies and desktop procedures.
  • Provides coaching, education and care management during the assessment and if specific criteria is met, initiate referral to complex care management.
  • Completes comprehensive assessments of members per regulated timelines and determines who may qualify for case management based on clinical judgment, changes in member's health or psychosocial wellness, and triggers identified in the assessment.
  • Facilitates care coordination across the care continuum (home, hospital, home health, or nursing facility), identifies community resources and makes referrals as appropriate.
  • Documents accurate and complete data for all member contacts in the appropriate database.
  • Develops and implements a case management plan in collaboration with the member, caregiver, physician and/or other appropriate healthcare professionals and member's support network to address the member needs and goals.
  • Performs ongoing monitoring of the care plan to evaluate effectiveness of interventions for goal achievement, suggest changes as needed and document updates accordingly.
  • Maintains the members' right to privacy and protects Homebase Medical operations by keeping information confidential.
  • Contributes to team effort by accomplishing related results as needed.
  • Actively support the achievement of Homebase Medical’s Vision and Goals.
  • Other duties as assigned.

   Qualifications and experience: 

  • Associate's Degree Nursing or higher from an accredited School of Nursing or a 4 Year college degree in Nursing 
  • CA RN license in good standing required 
  • Bilingual Mandarin speaking ability required 
  • 6+ months of clinical nursing experience
  • 3-5 years clinical experience with geriatric population (Acute, Ambulatory care, SNF and/or LTC) preferred
  • Medicare/Medi-Cal experience in managed care environment preferred

  • 2+ years of case management experience in a medical group, IPA and/or HMO setting preferred

  • Basic knowledge of related NCQA standards, CMS and DHCS regulations preferred

  • Navigate and access multiple software systems without error

  • Proficient in MS Office Suite (40 WPM)

  • Medical and clinical terminology conversant

  • Meets core competencies and clinical skills set for entry level

  • Excellent written and oral communication skills, as well as strong interpersonal, critical thinking and analytical skills

  • Excellent organizational, decision-making and multi-tasking skills

  • Demonstrates customer focus; proactively finds ways to exceed customer needs.
