This role is part time up to 35 hours a week.

Requirements

As a Jaan Health Patient Coordinator you will be working directly with Phamily clients to reach, engage, and inform their patients about the benefits of Phamily-enabled care programming. Patient Coordinators are on the front lines of patient engagement speaking with hundreds of patients per week.

Responsibilities and activities will include:

  • Daily patient outreach and engagement (through Phamily and phone calls)
  • Ongoing communication and coordination with client care teams and providers
  • Diligent record-keeping of patient interactions
  • Ad-hoc projects to help improve the patient coordination process
  • Daily coordination with the Customer Success team to enable increased customer benefit

Requirements to apply:

  • Excellent written and verbal communication ability - specifically over the phone
  • Interest in performing high-volume, high-quality patient outreach daily
  • Previous experience working in a clinical setting i.e. hospital, doctor’s office, health center, etc.
  • Cold-calling experience (preferred)
  • A valid Certified Medical Assistant (CMA) or Licenced Practical Nurse (LPN) license (preferred)
