Job Description

The Operations Coordinator for Behavioral Health is crucial in providing support and assistance to individuals in the behavioral health care system. As an Operations Coordinator for Behavioral Health, your primary responsibility is to work as part of a multidisciplinary care team to improve outcomes and cost-efficiency while respecting the perspectives and preferences of individuals in the behavioral health care system. Your role involves outreach, assessment, support, and empowerment of individuals to promote recovery and well-being.

What You'll Do:

  • Outreach: Identify and reach out to members who are considered high risk, high cost, or high utilization cases within the behavioral health care system.

  • Proactive Intervention: Encourage proactive interventions to limit expenses and promote positive outcomes for individuals in the care system.

  • Perform and document assessments of members' support needs, and share findings with the Behavioral Health Case Manager as part of routine collaborative efforts.

  • Ongoing Member Contact: Maintain ongoing contact and support for members as recommended by the Behavioral Health Case Manager. This includes scheduled follow-ups, progress assessments, and appropriate contact frequency based on member acuity.

  • Empowerment and Support: Provide empowerment skills and support to individuals in their journey towards successful recovery living. Assist them in identifying and accessing community and other resources, advise on processes, and encourage follow-through with proposed resolutions.

  • Referrals: Make referrals as appropriate and document them with supporting documentation.

  • Daily Life Assistance: Support individuals in their daily life decisions and the resolution of minor problems.

  • Community Engagement: Facilitate social opportunities in the community and encourage participation in community activities to foster healthy relationships.

  • Team Collaboration: Work collaboratively as part of a care team and actively participate in care team meetings.

  • Task Management: Manage task lists and daily lists of outreaches to ensure that responsibilities are met.

  • Care Plans: Create care plans that include SMART (Specific, Measurable, Achievable, Relevant, Time-bound) goals, document them in appropriate records.

Requirements:   

  • High school diploma or GED

  • 0-2 years of experience in a related field.

  • Experience with substance abuse treatment in either utilization management or direct care required for consideration.

Salary Range

$41,600.00 - $51,700.00
