As a TELUS Health MyCare Case Management Nurse Practitioner you will be a part of the Complex Care and Case Management team and you'll have be responsible for the provision of high quality primary and episodic healthcare services and work in an interdisciplinary collaborative model with Nurse Practitioners, Family Physicians, Nurses, Specialists, Dietitians and other allied health team members.

Complex Care and Case Management - As part of our interdisciplinary team, our case management Nurse Practitioners provide care for patients referred to our complex case management team.

As a TELUS Health MyCare Nurse Practitioner you will:
  • Practice to full scope (video - based, primary care) by assessing, diagnosing, and treating episodic and chronic conditions while supporting patients through their healthcare journey
  • Complete detailed chart reviews in conjunction with comprehensive history taking for patients referred to the team
  • Identify available community based resources and provide support to help link patients to these resources
  • Provide exceptional, safe, high quality, and evidence-informed primary & episodic care through assessment, ordering diagnostic tests, prescribing pharmacological and non-pharmacological therapies, and initiating specialist referrals
  • Provide regular patient follow up to promote health and educate on disease prevention and management
  • Collaborate with other nurse practitioners, registered nurses, physicians and the allied health team members to foster a team-based, interdisciplinary approach
  • Contribute to care plans, determining the cadence of clinically appropriate assessments within the team to meet individual needs of patients
  • Making internal and external referrals as appropriate
  • Optional: For those who wish to combine virtual and in-person care, we also have part-time hours available in clinics in Vancouver and Victoria for patients triaged to clinic from our episodic virtual service.

What do you need to apply?
  • Masters of Nursing - Family Nurse Practitioner degree (MN-NP), or equivalent, post-Master’s diploma - Nurse Practitioner (PMD)
  • Current registration as a practicing Nurse Practitioner with the British Columbia College or Nurses and Midwives (BCCNM)
  • Nurse practitioner category: Family OR Adult
  • Successful completion of the OSCE (Objective Structured Clinical Examination)
  • Active, unrestricted provincial license(s) with required prescribing and/or lab numbers
  • Experience in chronic disease prevention and management preferred
  • Additional experience in home and community based care is an asset!
  • Excellent communication and interpersonal skills to collaborate effectively with team members and patients
  • Ability to provide technology-supported care, computer competency and familiarity with EMRs
  • Comfortable with mobile technology (video calling), or the ability and desire to learn!
  • Be able to commit minimum of 8 hours per week
