Job Overview

Join our esteemed organization as a Bilingual Emergency Medical Technician (EMT) specializing in Korean and English. In this pivotal role, you will enhance the healthcare journey for patients by fostering strong connections with their primary care providers, ultimately promoting improved health and wellness.

Key Responsibilities

  • Collaborate remotely with the medical practice staff and Case Management team to effectively manage a defined patient population.
  • Operate independently yet cohesively as a vital member of the team, extending the capabilities of the medical practice.
  • Assist patients in navigating their health challenges by addressing inquiries, implementing impactful care plans, and bridging gaps in care, while escalating medical concerns as needed.
  • Accurately document patient medical histories and issues in the electronic health record (EHR) system.
  • Facilitate patient care through a comprehensive understanding of referral and prior authorization processes.
  • Build and sustain professional relationships with assigned medical practices and internal teams.
  • Meet designated Key Performance Indicators (KPIs) to drive positive health outcomes and minimize healthcare costs.
  • Engage with high-risk patients utilizing effective communication tools, including calls, texts, and emails.
  • Escalate medical concerns appropriately to the Health Guide Registered Nurse or other healthcare professionals.
  • Occasionally travel to support primary care physician offices as required.
  • Adhere strictly to security policies to safeguard Protected Health Information (PHI) and company intellectual property.

Required Skills

  • Bilingual proficiency in Korean and English (native or fluent).
  • Certification as a Medical Assistant or certified EMT.
  • A genuine passion for patient coordination and dedication to supporting individuals with complex medical needs.
  • Empathy and a willingness to develop skills in motivational interviewing and behavioral change management.
  • Exceptional communication abilities, with proficiency in documenting patient interactions accurately.
  • Familiarity with medical terminology and the operation of primary care practices.
  • Knowledge of social determinants affecting health outcomes.
  • Proficient in utilizing electronic health records (EHRs) and various software applications.
  • Strong aptitude for managing competing priorities while maintaining attention to detail.
  • Experience with EHR systems such as EPIC, Athena, or Cerner is beneficial.
  • Proven success in a remote work environment, demonstrating independence and collaboration within a care team.
