Job Overview

We are seeking passionate and dedicated Emergency Medical Technicians (EMT) who are bilingual in Korean and English to join our dynamic team. This remote position enables professionals to play a crucial role in connecting patients with their primary care providers, thereby enhancing overall health and wellness. As an EMT, you will guide individuals through their healthcare journeys, resolve challenges, and improve communication with medical practices.

Key Responsibilities

  • Collaborate remotely with medical practice staff and our Case Management team to support a specified patient demographic.
  • Operate autonomously while contributing effectively as a collaborative team member, serving as an extension of the medical practice.
  • Facilitate better health outcomes for patients by addressing inquiries and concerns, connecting them with healthcare resources, implementing impactful care plans, and closing care gaps. You will escalate medical issues when necessary and direct patients to appropriate resources, including specialty care and community organizations.
  • Accurately maintain records of patients' medical histories and issues within the electronic health record (EHR).
  • Comprehend the referral and prior authorization processes to enhance patient care.
  • Develop and nurture professional relationships with assigned medical practices and within our organization.
  • Achieve designated Key Performance Indicators to promote positive health outcomes while managing medical expenses.
  • Engage high-risk patients through effective communication strategies, including calls, texts, portals, and emails.
  • Appropriately escalate medical concerns to the Health Guide Registered Nurse, Primary Care Physician, or leadership.
  • Occasionally travel to assist primary care physician offices.
  • Adhere to all security policies and procedures to safeguard Protected Health Information (PHI) and company intellectual property.

Required Skills

  • Bilingual proficiency (Korean/English) is required.
  • Certification as a Medical Assistant or as a certified EMT is essential.
  • A deep commitment to patient coordination and serving individuals with multifaceted medical and social needs.
  • Empathy and a willingness to cultivate skills in motivational interviewing, critical listening, and behavioral change management to enhance patient engagement.
  • Exceptional communication abilities and proficiency in accurately documenting patient interactions using software.
  • Familiarity with medical terminology and the operations of primary care practices.
  • Knowledge of social determinants of health.
  • Proficient in documenting within electronic health records (EHRs) and utilizing care management analytics databases, along with various software programs.
  • Strong capability to prioritize tasks while remaining detail-oriented and focused.
  • Familiarity with EHR systems such as EPIC, Athena, and Cerner.
  • Excellent written, visual, and verbal communication skills.
  • Proven ability to track multiple tasks and ensure timely completion.
  • Capability to work independently as part of a patient's overall care team under the supervision of licensed clinical professionals.
  • Demonstrated success in a remote work environment.

Compensation And Benefits
