Job Overview

We invite you to become a vital part of our dedicated team, where you will play a crucial role in bridging the gap between patients and their primary care providers, fostering overall health and wellness. As a Bilingual Emergency Medical Technician (EMT) working remotely, you will engage with patients, facilitate their healthcare journeys, and enhance the communication pathways within their medical practices.

Key Responsibilities

  • Collaborate remotely with medical practice staff and our Case Management team to effectively manage a designated patient population.
  • Function independently yet remain an integral part of the team, acting as an extension of the medical practice.
  • Assist patients in achieving optimal health outcomes by addressing their inquiries, connecting them with necessary medical practices, implementing effective care plans, and closing care gaps. This includes escalating medical issues as appropriate and guiding patients toward relevant resources, including specialized care and community services.
  • Accurately document patient medical histories and concerns in the electronic health record (EHR).
  • Understand and navigate the referral and prior authorization processes to facilitate seamless patient care.
  • Cultivate and maintain trusting, professional relationships with assigned medical practices and within our organization.
  • Meet designated Key Performance Indicators (KPIs) to promote positive patient health outcomes while minimizing healthcare costs.
  • Engage high-risk patients through effective communication methods, including calls, texts, portals, and emails.
  • Appropriately escalate medical concerns to the Health Guide Registered Nurse, Primary Care Physician, or leadership.
  • Occasionally travel to support primary care physician offices as needed.
  • Adhere to all security policies and procedures to safeguard Protected Health Information (PHI) and company intellectual properties.

Required Skills

  • Bilingual in Korean and English - Native or fluent proficiency is essential.
  • Certification as a Medical Assistant or as a certified EMT is mandatory.
  • A strong commitment to patient coordination and serving individuals with complex medical and social needs.
  • Empathetic disposition and a willingness to develop skills in motivational interviewing, active listening, and behavioral change management to enhance patient engagement.
  • Strong communication skills with the ability to document patient interactions accurately and efficiently using software.
  • Familiarity with medical terminology and the operations of primary care practices.
  • Knowledge of social determinants of health.
  • Proficient in using electronic health records (EHRs) and care management analytics databases, alongside various software programs.
  • Exceptional ability to balance competing priorities while remaining detail-oriented.
  • Experience with various electronic health record systems such as EPIC, Athena, and Cerner.
  • Excellent written, visual, and verbal communication skills.
  • Ability to manage multiple tasks and ensure timely completion.
  • Capability to work independently as part of a comprehensive care team, under clinical supervision.
  • Proven success in a remote work environment.

Qualifications

  • Certification as a Medical Assistant or certified EMT is required.
  • Experience in patient care coordination and collaboration with diverse medical teams is advantageous.

Compensation And Benefits

The base pay range for this role starts at $24+ per hour, paid bi-weekly. Compensation may vary based on factors such as skill set, years of experience, education, location, and certifications.
