Job Overview

We invite you to become an integral member of our dedicated team, where you will play a crucial role in bridging the gap between patients and their primary healthcare providers. Your efforts will contribute significantly to enhancing overall health and wellness through effective patient engagement and communication.

Key Responsibilities

  • Collaborate remotely with healthcare staff and our Case Management team to manage a designated patient population.
  • Operate autonomously while being a valuable team player, serving as an extension of the medical practice.
  • Assist patients in reaching improved health outcomes by addressing their inquiries, connecting them with their healthcare providers, implementing effective care plans, and closing care gaps. Escalate medical concerns as necessary and direct patients to suitable resources, including specialty care and community organizations.
  • Accurately document patients' medical histories and issues in the electronic health record (EHR).
  • Understand and manage the referral and prior authorization processes to facilitate patient care.
  • Build and sustain trusting, professional relationships with assigned medical practices and within our organization.
  • Meet established Key Performance Indicators to promote positive patient health outcomes while minimizing medical costs.
  • Engage high-risk patients using effective communication strategies, including calls, texts, portals, and emails.
  • Appropriately escalate medical issues to the Health Guide Registered Nurse, Primary Care Physician, or leadership as needed.
  • Occasionally travel to assist primary care physician offices.
  • Comply with all security policies and procedures to safeguard all Protected Health Information (PHI) and company intellectual properties.

Required Skills

  • Bilingual (Korean/English) - Native or fluent proficiency is essential.
  • Certification as a Medical Assistant or as a certified EMT is mandatory.
  • A genuine passion for patient coordination and a commitment to serving individuals with multifaceted medical and social needs.
  • Empathy and readiness to cultivate skills in motivational interviewing, critical listening, and behavioral change management to enhance patient engagement.
  • Outstanding communication skills with the ability to document patient interactions accurately and effectively using software.
  • Familiarity with medical terminology and the operations of primary care practices.
  • Knowledge of social determinants of health.
  • Proficiency in documenting within electronic health records (EHRs) and utilizing care management analytics databases, along with various software applications.
  • Strong capability to balance competing priorities while remaining detail-oriented and focused.
  • Acquainted with various electronic health record systems such as EPIC, Athena, and Cerner.
  • Exceptional written, visual, and verbal communication skills.
  • Ability to manage multiple tasks and ensure timely completion.
  • Capability to work independently as part of a patient's overall care team under the supervision of licensed clinical professionals.
  • Proven success in a remote work environment.

Compensation And Benefits

The base pay range for this role starts at $24+ per hour, paid bi-weekly. Compensation may vary based on skills, years of experience, education, location, and certifications.
