Job Overview

Join our esteemed organization as an Emergency Medical Technician (EMT) and play a pivotal role in enhancing the connection between patients and their primary healthcare providers. In this remote position, you will be instrumental in guiding patients through their healthcare journeys, addressing their inquiries, and fostering effective communication with their medical teams.

Key Responsibilities

  • Collaborate remotely with medical practice staff and the Case Management team to manage a defined patient population.
  • Operate independently while serving as an integral member of the healthcare team and an extension of the medical practice.
  • Facilitate improved health outcomes by addressing patients’ questions and concerns, connecting them with their medical practices, implementing comprehensive care plans, and efficiently closing care gaps. Escalate medical issues as needed and direct patients to necessary resources, including specialty care and community organizations.
  • Meticulously document patient medical histories and issues in the electronic health record (EHR).
  • Understand referral processes and prior authorizations to streamline patient care.
  • Build and sustain professional relationships with assigned medical practices and within our organization.
  • Meet established Key Performance Indicators to promote favorable patient health outcomes while minimizing healthcare costs.
  • Engage high-risk patients through varied communication methods, including calls, texts, portals, and emails.
  • Appropriately escalate medical concerns to the Health Guide Registered Nurse, Primary Care Physician, or management.
  • Occasionally travel to assist primary care physician offices as required.
  • Adhere to security policies and procedures to safeguard Protected Health Information (PHI) and company intellectual properties.

Required Skills

  • Bilingual (Korean/English) - Native or fluent proficiency is essential.
  • Certification as a Medical Assistant or as a certified EMT is mandatory.
  • A strong passion for patient coordination and dedication to serving individuals with complex medical and social needs.
  • Empathy and a commitment to developing skills in motivational interviewing, critical listening, and behavioral change management to enhance patient engagement.
  • Excellent communication skills with an aptitude for accurate and efficient documentation of patient interactions using software.
  • Familiarity with medical terminology and primary care practices.
  • Understanding of social determinants of health.
  • Proficiency in documenting within electronic health records (EHRs) and utilizing care management analytics databases, along with various software programs.
  • Strong ability to manage multiple priorities while remaining detail-oriented.
  • Familiarity with different electronic health record systems such as EPIC, Athena, and Cerner.
  • Exceptional written, visual, and verbal communication skills.
  • Proven ability to monitor multiple tasks and ensure timely completion.
  • Capability to work independently as part of a patient's comprehensive care team, under the guidance of licensed clinical professionals.
  • Demonstrated success in a remote work setting.

Qualifications

  • Certification as a Medical Assistant or as a certified EMT is required.
