Position Overview:

The Case Management Assistant III (CMS) communicates by telephone or electronically with potential or current patients enrolled in Sutter Anticoagulation Program. Duties include outreach to and enrollment of new members, discussions with patients already enrolled in Sutter Anticoagulation Program to provide lab results, medication information and education as directed by an RN Case Manager, and to triage incoming patient calls to the program phone lines. Other duties include extensive use of the Epic electronic health record, data entry and report generation and communication with a variety of medical providers. The CMS works independently and collaboratively as a member of self-directed work teams consisting of both clinical and non-clinical staff positions. A CMS is also capable of taking a lead role in certain processes or projects.

Job Description:

EDUCATION:

  • Equivalent experience will be accepted in lieu of the required degree or diploma.
  • Associate's: AA in Business/Health related field


TYPICAL EXPERIENCE:

  • 1 year recent relevant experience.


SKILLS AND KNOWLEDGE:

  • Oral and written communication skills.
  • Interpersonal and time management skills
  • Ability to work effectively in a fast pace environment with rapidly shifting priorities and competing demands.
  • Ability to work independently with a minimum of direction. Ability to exercise discretion and prioritize tasks, seeking input as indicated.
  • Intermediate PC skills and word processing skills required.

Pay Range is $28.69 to $37.28 / hour
