Your missions

Description

We aim to provide quality and compassionate care in our communities, and we are excited to offer a \(500 sign-on bonus, with an additional \)500 for qualified Mental Health Professionals. This is a full-time, benefit-eligible position that involves working Monday to Friday from 10:00 am to 7:00 pm, with a rotating Saturday office schedule (10:00 am - 2:00 pm) every 5 weeks.

Essential Functions

  • Provide individualized and group education services using a motivational, stage-matched approach, focusing on the client's recovery/resiliency goals.
  • Assist clients in identifying their strengths and needs, contributing to the development of Wellness Plans.
  • Deliver services from a holistic perspective, incorporating coaching on the Eight Dimensions of Wellness.
  • Support clients in completing goals outlined in Wellness Plans, making necessary updates as needed.
  • Aid clients in accessing public services, such as financial, housing, or food assistance.
  • Maintain contact with hospitalized clients, aiding in discharge planning and developing aftercare plans.
  • Provide education and skill-building related to daily living skills, employment, education, and symptom management.
  • Assist clients in developing personal support systems, including collaboration with family members and recovery communities.
  • Offer individual assistance in accessing necessary services, including accompanying clients to appointments.
  • Follow up with clients regarding appointments and necessary documentation.
  • Re-engage consumers with unexcused absences and promote recovery efforts.
  • Meet service provision expectations aligned with client needs and company guidelines.
  • Complete documentation and paperwork within required timeframes.
  • Collect and process urine drug screens.

Integrated Health Team

  • Communicate actively with the integrated team, including psychiatrists and other healthcare providers, about client wellness.
  • Gather input regarding clients' Wellness Plans from Integrated Health Team providers.
  • Identify services needed and facilitate hand-offs to those services.
  • Make referrals to additional services or providers as required.
  • Serve as a liaison to others involved in client treatment, such as family members and legal entities.

Miscellaneous

  • Seek culturally competent services that meet the unique needs of clients and families.
  • Provide transportation assistance to clients as clinically indicated; driving is an essential function of this position.
  • Perform other duties as assigned.

Qualifications

  • High School Diploma or GED required; an Associate’s or Bachelor’s degree in a human service-related field preferred.
  • If no degree, the following experience is required:
    • Four (4) years of experience or equivalent combination of higher education and experience if High School/GED is the highest education.
    • Two (2) years of experience or equivalent combination if an Associate’s degree is the highest education.
    • One (1) year of experience if a non-human service Bachelor’s degree is the highest education.
  • All experience must include working with individuals with psychiatric, substance use, or developmental disabilities.
  • A valid driver’s license and agency-established minimum automobile coverage are required.
