Position Description:

Circadia Health is revolutionizing remote patient monitoring for post-acute care and skilled nursing facilities. We are seeking a dedicated and experienced Nurse Practitioner to join our dynamic team. This role will be pivotal in assessing patients health status and providing care plans while utilizing cutting-edge technology to monitor and improve the health outcomes of our long-term care patients. As a Virtual Nurse Practitioner with Circadia Health, you will play a crucial role in our mission to enhance patient care through innovative remote monitoring solutions. You will be responsible for performing the initial patient visit, developing individualized care plans and collaborating with our interdisciplinary team to ensure the highest standards of care.

Responsibilities:
  • Perform comprehensive assessments of patients using telehealth platforms to gather medical histories and evaluate current health conditions.
  • Create individualized care plans that specify health monitoring parameters and outline achievable health goals for each patient.
  • Maintain accurate and timely documentation of patient interactions, assessments, education, and care plans within the EHR system. Ensure all records are compliant with policies and regulatory standards. 
  • Assist in training other providers on remote monitoring workflow and best practices.
  • Collaborate with Skilled Nursing physicians, Circadia physicians, nurses, and other healthcare professionals to provide a comprehensive approach to patient care.
  • Participate in multidisciplinary team meetings to discuss and coordinate patient care strategies.

  • Qualifications:
  • Certified and licensed as a Nurse Practitioner in California
  • Additional state licenses including but not limited to NY, NJ, or MI is a plus
  • Medicare participation and ability to have company bill for services on your behalf
  • Certification from ANCC (or equivalent) as an Adult, Family, Geriatric and/or Acute Nurse Practitioner
  • 1+ years of Nurse Practitioner Experience working with complex adult populations
  • 1+ years of telehealth experience
  • Ability to practice independently with little or no supervision 
  • Comfort using technology such as Google Suite, multiple EMRs, Slack 
  • Comfortable in a dynamic and always evolving start up environment
  • English required, Spanish preferred
