We are a medical practice dedicated to improving the health care experience for seniors as well as our providers. We founded Heyday Health because we saw the health system leaving our older loved ones behind and were driven to create a better solution. Our practice is being built from the ground up with the mission of transforming how, where, and when we deliver care to achieve exceptional outcomes.
We are looking for a professional Nurse Practitioner to join our team of dedicated, patient-oriented providers. The role will include:
- Providing at-home medical care for intake visits, managing chronic conditions and responding to acute concerns (estimated 50% of patient interactions)
- Delivering excellent medical care to seniors through video, phone, email, and chat communications (estimated 50% of patient interactions). This work can be done from home and would not require travel to an office
- Maintaining accurate patient records, administering intake and follow-up appointments, and communicating with patient family members and caregivers as needed
- Working closely with the Heyday Health team of physicians and health promoters to advocate for every patient and ensure the highest quality care
- Reviewing your panel of patients with your physician supervisor as well as the rest of the team to identify opportunities to improve the quality of care and reduce avoidable hospitalizations and ER visits
We think you’d be a great fit if you have an active and unrestricted Ohio Nurse Practitioner license and:
- Fantastic communication skills and a passion for building relationships with patients and care teams
- Enthusiasm for contributing to and developing innovative models for patient care
- Valid driver’s license and reliable personal vehicle that can be used for at-home patient visits
- Experience with telehealth and commitment to learning how to use various digital health platforms
- Comfort with using electronic health records and other computer systems
- Dedication to doing the right thing for patients and providing compassionate care
