Job Description: 

 

Are you an enthusiastic, patient-centered nurse who lives in South Carolina? Join our dedicated team of Care Managers that specialize in providing comprehensive care for aging adults. Our team works from home and provides chronic care management services for patients who are living in skilled nursing, assisted living, and other long-term care facilities. 

 

Key Responsibilities: 

 

  • Perform comprehensive chart reviews, reconcile medications, and identify potential issues that need to be addressed. 
  • Develop and implement personalized care plans tailored to each patient's needs. 
  • Monitor and assess patient progress, adjusting care plans as necessary to ensure optimal outcomes. 
  • Collaborate with other healthcare providers and facilities to coordinate patient care and ensure a cohesive approach. 
  • Document all patient interactions, assessments, and care plans accurately in the electronic health record system. 
  • Ensure compliance with all healthcare regulations and standards of practice. 

 

Requirements: 

 

  • Must be a licensed nurse LPN in the state of South Carolina in good standing.   
  • Strong communication and interpersonal skills. 
  • Proficient in using computers and various software applications. 
  • Self-motivated and able to work independently in a remote setting.

Preferred Qualifications: 

 

  • Familiarity with chronic care management and geriatric care.
