Resource Specialist. This is a 03-04+ Months Contract opportunity with long-term potential and is located in Florida(Hybrid Please review the job description below and contact me ASAP if you are interested.Job ID: 24-41453 Pay Range: $26 - $30/hour.

Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location Key Responsibilities: This position will be field based and will provide care management to assist with coordination of care, including assessing for health care needs, referring to health plan providers, providing education on benefits/expanded benefits, and provide community resources to address social determinants of health needs.

The caseload is 100 members

This position will document in an electronic health record and will be required to document outreach notes, complete assessments, and complete a care plan on each member

Compliance with care management policies/procedures including outreach and documentation expectations

Individuals will be audited on a monthly basis to ensure charts are in compliance

Key Requirements and Technology Experience: Skills-Pediatric case management experience, Experience connecting families/children to local resources for various services including employment, housing, independent living, justice, foster care, etc. and Health plan experience. Dependency Case Manager. Child Protective Investigator. DCF Navigator. Adoption Case Manager. Targeted Case Manager

Community Agency Worker.

Foster Care And/or Adoption Experience. Education Requirement

Bachelor’s Degree

Software Skills Required: Microsoft Office- Outlook, Word, and Excel

Note-Individuals will work from their home and will also be required to go into the field.
