The Care Management Extender is an integral part of the Tailored Care Management team approach to integrated care for behavioral health and medical care by supporting the members through collaborative efforts that improve member outcomes with activities such as linking members to medication management, coordinating physical and behavioral health care needs, educating members, building trust between members and their practitioners, supporting and connecting members to community recovery programs, and enhancing communication and continuity of care.
Essential Duties:
- Perform general outreach, engagement, and follow-up with members to assist them in making informed choices regarding their care and services and other life decisions;
- Provides various services and supports to emphasize personal safety, self-worth, confidence, connection to the community, boundary setting, planning, self-advocacy, personal fulfillment, development of social support and effective communication skills;
- In conjunction with the care management team, engage in various skill-building activities such as learning how to obtain and maintain stable housing, manage finances, increase employment readiness skills, interact with others in private and public settings, maintain healthy, non-violent relationships, engage in safe, non-medical ways to control modifiable risk factors that impact the person's health (e.g., smoking, drug and alcohol use, obesity, etc.) and learn skills to increase motivation and actively address health conditions such as diabetes, heart disease, COPD, etc.
- Coordinating services/appointments (e.g., appointment/wellness reminders, providing or arranging transportation).
Eligible Candidates Must Have:
- A high school diploma or equivalent
- Must be 18 years or older.
- Be a person with lived experience with a behavioral health condition and a Certified Peer Support Specialist
or
- A parent or guardian of an individual with a behavioral health condition and has at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual
or
- Have two years of paid experience performing the types of functions described in the “Extender Functions” section below, with at least one year of paid experience working directly with the Tailored Care Management eligible population.
The Care Management Extender is a semi-remote working position and will include a combination of office, home and community based work.
Starting Salary Range: $35,000 - 40,000*
*commensurate with experience
