About the role:
Foodsmart is seeking Registered Dietitians (RDs) to support the comprehensive nutrition care mission of Foodsmart Nutrition Network, which is grounded in removing the financial barriers in accessing and affording healthy food. Our Foodsmart RDs leverage Foodsmart's digital platform to provide a broad range of nutrition and health-related consultations, ultimately helping our members stretch their food dollar, eat healthier, and reduce the risk of chronic disease.
In this role, you'll have the ability to truly change the lives of the members we serve, many of whom are Medicaid members and from underserved communities. You will tackle the root causes of food and nutrition insecurity. This means that you will provide MNT in multiple capacities, including supporting your patient in accessing and using SNAP benefits, affording and buying groceries digitally on Foodsmart's marketplace, and building healthier eating and shopping habits for life. Foodsmart removes the barriers that prevent so many people from eating healthier--so that you, as an RD, can have a major impact on each and every member.
Role ExpectationsYou are completing at least 6 follow-up visits with your patientsYou are receiving an NPS of 80% and aboveYou find value in developing lasting relationships with your patients to support them towards sustainable behavior change and improved health outcomesYou will:Provide individualized medical nutrition therapy (MNT) via remote sessions to patients with a variety of health and lifestyle needsDocument care in electronic medical record according to Nutrition Care ProcessAssist patients in getting the most utility out of Foodsmart's digital platformManage a panel of clients/patients with a variety of chronic health conditions and backgroundsCommunicate effectively and empathetically with clients/patients through HIPAA-compliant video calls and electronic messagingYou have:Active credentialing as a Registered Dietitian by the Commission on Dietetic Registration (CDR) and Licensed DietitianMust be a Certified Licensed Dietitian in New YorkWillingness to apply for additional licensure in at least one other stateMinimum of one (1) to two (2) years of professional experience in providing nutritional counselingAbility to work in the U.S.
