About The Role:

The Registered Nurse (RN) - Transitions of Care plays a critical role on the Galileo care team. This individual will collaborate with our multidisciplinary team to ensure patients transition safely and successfully from an inpatient setting back to their homes.

This is a remote, full-time position at 40 hours/week. The core working hours will be between 9:00am-5:30pm EST, Monday through Friday with a requirement of 1 weekend day coverage per month.

Here’s what you’ll do:
  • Telephonic outreach to patients newly discharged for medical or mental health conditions to assess symptoms, barriers to medication adherence, safety concerns and social needs
  • Assess risk of BH-related emergencies and execute appropriate protocol with interdisciplinary team
  • Recognize the unique needs of patients with co-morbid medical and BH concerns (example: differential diagnosis, acute psychiatric decompensation, severity of withdrawal symptoms) and triage appropriately
  • Coordinate all aspects of the care transition from hospital/ facility to home
  • Provide compassionate and empathetic care to patients and their families post-discharge, to support safe transitions
  • Collaborate with primary care, behavioral staff and other support teams to provide excellent patient care
  • Additional, ad hoc tasks as dictated by capacity and organization needs (such as gap closure outreach, medication adherence assessment and urgent care triaging)

  • 2+ year of transitions of care experience
  • NY RN licensure required
  • Comprehensive knowledge and experience providing care for patients with complex medical and behavioral health conditions
  • Highly culturally competent with diverse populations
  • Tech savvy and effective communicators with EMR systems
  • Are highly comfortable with using technology and various applications to deliver care to patients virtually
  • Eager to offer suggestions for improvements and able to problem solve on the go
  • Excellent written and verbal communication skills
  • Thrive in a flexible start-up environment where changing workflows, systems, and tools may be frequent.
  • Bi-lingual in English and Spanish a bonus!
Physical Requirements:

Employee must be able to meet the following requirements with or without a reasonable accommodation: This is primarily a sedentary position. Physical requirements may include lifting up to 10 pounds, manual dexterity, near/far visual acuity, keyboarding, the ability to hear, understand, and distinguish speech, sitting, standing, walking, and screen usage 8 or more hours per day.

Compensation Range:

$46/hr (regular hours) and $55/hr (premium hours)
