We are looking for dedicated Registered Nurses and Licensed Practical Nurses to join our team as we redefine the healthcare experience for individuals living with chronic illnesses. Our approach revolutionizes remote care management, ensuring that vulnerable individuals are never alone and receive consistent support as they navigate their health challenges.

Your Role

As a Chronic Care Manager, you will provide compassionate care and patient education while enjoying the flexibility of working from home. This position allows you to skip the daily commute and be present for your family, all while building meaningful connections with your patients.

Work Schedule

  • Monday to Friday, 8:00 AM - 4:30 PM PT or 11:00 AM - 7:30 PM EST
  • After your first 6 months, customize your schedule to fit your lifestyle:
    • 4 days x 8 hours
    • 4 days x 9 hours
    • 4 days x 10 hours

A Day in the Life of a Chronic Care Manager

  • Conduct regular check-ins with patients to discuss changes, follow up on appointments, and clarify new instructions from their doctor.
  • Build meaningful connections and understand patients’ unique personalities and challenges.
  • Establish and nurture trust with new patients and their families.
  • Act as a vital link within the patient’s care team, communicating seamlessly with healthcare providers and in-office staff.
  • Guide patients in prioritizing their health and understanding their conditions.
  • Advocate for patients by providing prompt answers to their medical concerns.
  • Utilize technology to manage and coordinate care, from gathering resources to setting care goals.
  • Use Electronic Medical Records (EMR) Systems to review recent office visits, liaise with the care team, and accurately document all actions taken for patients.

Are You a Great Fit? Your Qualifications

  • Compassion: You provide heartfelt care to patients.
  • Licensing: You are an RN or LPN licensed in any U.S. state in the Pacific Time Zone. If your role involves caring for out-of-state patients, we will cover your licensing fees.
  • Experience: Minimum of 4 years in nursing care for chronically ill patients; experience in home health or primary care settings is a plus.
  • Tech-Savvy: Comfortable learning and adapting to new technology tools.
  • Time Management: Proven skills in managing your time effectively, especially when working remotely.
  • Communication: Exceptional active listening skills and clear communication, both in writing and verbally.
