Your missions

 

What You Should Know About the LPN Care Team Coordinator position:

  • Full Time, Monday-Friday with the hours of 8:00am-4:30pm
  • Works within the interdisciplinary team to create comprehensive individualized plans of care for assigned patients. 

Qualifications:

Associate degree in medical related field, required, Active Licensed Practical Nurse Preferred

·         Minimum 2 years’ experience in a health care setting and understanding of the person-centered care philosophy required.  

·         Experience in Quality Initiatives strongly preferred.

·         Ability to create Quality Improvement activities as needed.

·         Customer service experience, or related experience, preferred.

·         Excellent clerical, organizational and computer skills including Word, Excel, and PowerPoint skills as well as communication skills - verbal and written.  

          Ability to effectively multitask, essential. Ability to work effectively in an interdisciplinary approach to patient care and follow a care plan.

·         Will work from home office but may be required to do some traveling for meetings and member visits.

·         CPR Certified preferred; required in orientation at hire.

·         Notary designation or intention to complete within six months of hire.

·         Valid Driver's License with Safe Driving Record with successful clearance of a driving record check

·         Ability to drive during daytime, nighttime, or inclement weather. This position requires travel to different work locations within the counties we serve.

·         State minimum automobile insurance coverage.

·         Successful clearance of a criminal background check
