Behavioral Health Care Manager - Remote
DIRECT ACCESS is looking for a bilingual medical assistant to provide remote care management support to patients that are primarily home bound.
Responsibilities include:
- Coordinate care needs for patients.
- Communicate with patient's care team, which may include physicians, family members, and other care providers.
- Document care management communications.
- Create and maintain a comprehensive, patient centered care plan.
- Utilize the care plan to assist patient with achieving their goals.
- Provide education for chronic and specialty disease conditions.
- Other duties as needed.
