Health Navigator

Position Summary:

The Health Navigator (HN) will provide daily care coordination, case management, coaching, consultation and intervention to patients with one or more chronic diseases. Overall duties include supporting clinicians in meeting chronic and preventive health needs of their patients; guiding patients in collaborative self-management for the prevention and management of chronic disease; linking patients to community resources and coordinating care provided in multiple practice settings across the care continuum. As part of an interdisciplinary care team, the HNK coordinates patient services, such as social work and mental health counseling, psycho-social support services, in-home support, skilled nursing, and home health.

Position Responsibilities:

  • Coordinate health care services for patients through assessment of their chronic conditions and/or health care needs.
  • Utilize the case management process to guide service delivery throughout the health care continuum to ensure quality care delivery is safe, timely, effective, efficient, equitable and patient-centered.
  • Conducts outbound care management phone calls and in-person patient visits during medical appointments and/or in their home.
  • Effectively collaborate with all members of the health care team to include members of the interdisciplinary care tea, the patient and their family members as appropriate.
  • Establish and monitor patient care plans.
  • Establish trusting relationships with patients, providers, and community. Tailor communications and interactions to ethnic and cultural diversity.
  • Facilitate appropriate screening for early detection.
  • Work in partnership with Patient Care Advocates to ensure orders, tests, procedures, and referrals are performed and communicated in a timely, effective, efficient and patient- centered manner.
  • Develop synergy between Health Navigator and Patient.
  • Engage patients (and their caregivers) in understanding and settling self-management plans in a culturally and linguistically appropriate manner.
  • Support patients in their health behavior change efforts.
  • Link patients to ancillary health services, community resources and public health.
  • Provide clinical documentation of services.
  • Maintain patient confidentiality.
  • Participate in collection of process and outcomes data and in all efforts pertaining to continuous quality improvement.
  • Maintain core proficiencies as defined by the Health Navigator Program.
  • Attend meetings, conferences and trainings related to the Health Navigation Program.
  • Other duties as assigned

Position Qualifications:

Required

  • 3 plus years as a licensed, registered nurse (RN).
  • Experience in health coaching, motivational interviewing
  • Must possess a valid North Carolina driver’s license and vehicle


Preferred

  • Experience in outpatient primary care, acute care, care coordination and case management
  • Relevant professional certification(s) such as Case Management, Disease Management, Health Coaching, Guided Care Nursing, Certified Diabetes Educator
