Position Purpose: Provide Palliative Care Program leadership, providers, staff, volunteers, patients, and families with travel information/plans, equipment, and supplies in a proactive and efficient manner consistent with the goals of the Palliative Care Program.
Essential Functions

Schedule appointments for providers, staff and volunteers with attention to location, prep, and drive time to ensure visits are made timely.

Ability to utilize software and/or tools in order to plan provider, staff, and volunteer visits as above for all areas served by the Palliative Care Program.

Assist Program with arranging and follow-up on delivery of DME, supplies, services and items as assigned.

Assist with setting up transportation if needed for patients.

Assist with requesting new hire equipment, supplies, and other assistance as needed. Follow-up and ensure with HR, IT and other departments that items are delivered. Maintain records of all equipment and re-usable supplies provided to each staff member.

Order and maintain medical and office supplies par levels for Palliative Corporate office space, Sanitas Medical Center hubs in each service area, remote worker homes, in collaboration with staff and purchasing.

Assist with scheduling, working with internal departments and external vendors, and ordering for Palliative Care internal and external community events.

Attend applicable Palliative Care Program Meetings.

Complete Sanitas and Palliative Program educational training as assigned.

Maintain confidentiality of PHI.

Plan for, participate and respond to events such as weather, emergency, traffic, safety and IT issues where Program staff may need assistance to navigate their day.

Evidence of essential leadership, communication, and customer service skills.

Able to work under-pressure, meet deadlines and results driven with a flexible supportive and collaborative approach.

Proficiency in communication technologies (computer, printer, fax, email, cell phone, etc.).

Highly organized with the ability to keep accurate notes and records. Track and provide reports on utilization of supplies and services supplied monthly.

Experience with health IT systems and reports is desirable.

Advanced knowledge of Excel is preferred.

Any combination of education, training, and experience which demonstrates the ability to perform the duties and responsibilities as described including related work experience.

Proficiency in medical terminology preferred

Ability to work with and serve a diverse workplace and patient population.
Other Languages: Spanish preferred
Supervisor Responsibilities: This position has no supervisory responsibilities.
Environmental Conditions:
Inside: Protection from weather conditions but not necessarily from temperature changes. The worker is subject to noise; there may be sufficient noise to cause the worker to shout in order to be heard above ambient noise level.
Outside: Employees may participate in outside events or activities. There may be exposure to weather conditions, temperature changes. There may be exposure to noise from music or crowds at events.
Physical Demands:
While performing the duties of this job the employee is regularly required to work standing up, walk, use hands to operate tools and equipment and must be able to exert regularly up to 10 pounds of force, frequently exert 30 pounds of force and occasionally exert 50 pounds of force to constantly perform the essential job functions. The employee will be frequently required to reach with hands and arms, bend, balance, kneel, crouch, crawl, push, and pull. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
