The Medical Group Physician will practice in an evidence-based environment, utilizing the resources of the Somatus model of care to deliver improved clinical care for patients with kidney disease. The primary focus of the clinical model is to provide a holistic, comprehensive care that is focused on achieving quality clinical outcomes, delaying disease progression, spending more time with each patient, and practicing patient-centered medicine. This position will partner with a collaborative team to expand this clinical practice with a uniquely innovative, value-based model focused on delivering high quality clinical care. This position will report to the National Medical Director.
Responsibilities
Essential Duties and Responsibilities:
- Provide the full spectrum of clinical care, including providing consultations, follow up visits, and urgent episodic care to patients with or at risk for kidney disease.
- Responsible for the provision of care and licensure in several states, providing direct care predominantly telehealth in addition to in-home face to face care on an asynchronous as well as longitudinal basis.
- Practice within a population health model which prioritizes robust and reliable provision of timely, proactive evidence-based clinical care to optimize patient health and address gaps in care.
- Provide the full spectrum of outpatient clinical care, including consultations, follow up visits, and urgent episodic care to patients with kidney disease where needed.
- Provide bridge care for members around transitions of care and can involve prescribing medication and ordering appropriate testing for diagnostics.
- Maintain an up-to-date, evidence-based clinical practice focused on management of CKD and ESKD, delaying kidney disease progression, comorbidity management, and education for optimal ESKD transitions where indicated.
- Oversee a local state-based team of nurse practitioners who provide direct virtual and in-home prescriptive clinical care under the Population Health Physician’s clinical supervision.
- Partner with a multidisciplinary team, including RN case managers, social workers, clinical pharmacists, registered dietitians, and patient health advocates, focused on delivering truly integrated, value-based care to nephrology practice. Ensure smooth communication and coordination of care for patients with established primary and specialty care relationships.
- Provide specialty expertise to support Somatus Medical Group, P.C.
- Responsible for driving the continuous improvement of care across the entire panel of patients including achieving reductions in avoidable utilization (e.g., avoidable ED, inpatient admissions,
- and hospital readmissions), as well as achieving top decline performance across measurable quality of care outcomes, patient activation, and patient satisfaction.
- Partner with a multidisciplinary team to help understand the medical, behavioral, and psychosocial needs of each patient, identify gaps in care and address barriers to achieving optimal health outcomes.
- Partner with nurse educators in providing robust disease education.
- Establish and maintain strong physician relationships with local physicians and health systems. Provide timely transitions of care visits and care, with a focus on keeping patients healthy at home and reducing unnecessary inpatient utilization.
- Provide comprehensive and timely communication to referring physicians and collaborate closely with other local treating physicians to ensure coordinated care with an ideal patient-centered experience.
- Oversee all program requirements including participation and responsibilities for quality assurance and quality improvement.
- Serve as collaborative physician to provide oversight to nurse practitioner(s) performing risk adjustment visits and/or patient care visits in relevant states.
- Provide ongoing coaching, feedback, and assistance to Somatus care team members for leadership and management support. Uses proven techniques of practice facilitation to influence and promote high-value healthcare. Identifies practice educational needs and matches them with available Somatus or industry resources.
- Credential with requested payers and provide updated information as needed to maintain that credential.
- Serve as an owner of one or more friendly professional corporations, such as Somatus Medical Group.
- Ability to travel and visit patients as needed.

Qualifications
- Graduate of an accredited medical school. M.D./D.O. degree is required.
- Completion of approved residency in internal medicine.
- Current state medical license without restrictions to practice and free of sanctions from Medicaid or Medicare. Willingness to become licensed in multiple states as company and patient needs require.
- Currently board certified or board-eligible in applicable specialty.
- Must be able to obtain licenses to practice medicine and to keep those licenses active and good standing in states identified by Somatus to support Somatus clinical and business needs.
- Minimum 2-3 years of post-graduate experience caring for complex adult populations.
Knowledge, Skills, And Abilities
- Willingness to serve as the medical director for Somatus direct patient care programs at Somatus locations, including newly created ventures, where needed.
- Effective oral and written communication skills, with an ability to communicate effectively and sensitively with patients, clinicians, and care teams in stressful situations.
- Ability to lead and influence colleagues and lead teams cross-functionally.
- Ability to work in a rapidly changing, matrixed environment.
- Demonstrated professional competence, strong computer skills and EHR competency.
