Job description
This position is fully remote with the exception of the initial onboarding / orientation and weekly meetings with supervisor. It pays $50 per month per client (2 telehealth / telephone visits each month which generally take 15-30 minutes each call). Each visit may be completed over the phone or via telehealth. There are currently a total of 72 clients on the caseload who need to be serviced monthly.
Responsibilities:
As a Registered Nurse Case Manager (aka Health Home Care Manager), you will be responsible for providing comprehensive and coordinated care to patients. Your primary duties will include:
- Conducting patient assessments and developing individualized care plans
- Collaborating with healthcare professionals to ensure the delivery of quality patient care
- Monitoring patient progress and making necessary adjustments to the care plan
- Educating patients and their families on disease management and treatment options
- Coordinating and facilitating communication between patients, healthcare providers, and other stakeholders
- Ensuring compliance with medical records documentation and HIPAA regulations
Qualifications and Experience:
To be successful in this role, you should have the following qualifications and experience:
- Valid RN license in the state of Maryland
- Experience in pediatrics, acute care, or behavioral health is preferred
- Strong knowledge of medical terminology and patient assessment techniques
- Familiarity with managed care principles and practices
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
